Refactor vfio_pci_mmap_huge_fault to take out the implementation
to map the VMA to the PTE/PMD/PUD as a separate function.
Export the new function to be used by nvgrace-gpu module.
Move the alignment check code to verify that pfn and VMA VA is
aligned to the page order to the header file and make it inline.
No functional change is intended.

Thanks to a device generating an ACS violation during bus reset,
lockdep reported the following circular locking issue:
CPU0: SET_IRQS (MSI/X): holds igate, acquires memory_lock
CPU1: HOT_RESET: holds memory_lock, acquires pci_bus_sem
CPU2: AER: holds pci_bus_sem, acquires igate
This results in a potential 3-way deadlock.
Remove the pci_bus_sem->igate leg of the triangle by using RCU
to peek at the eventfd rather than locking it with igate.

Add support for exporting PCI device MMIO regions through dma-buf,
enabling safe sharing of non-struct page memory with controlled
lifetime management. This allows RDMA and other subsystems to import
dma-buf FDs and build them into memory regions for PCI P2P operations.
The implementation provides a revocable attachment mechanism using
dma-buf move operations. MMIO regions are normally pinned as BARs
don't change physical addresses, but access is revoked when the VFIO
device is closed or a PCI reset is issued. This ensures kernel
self-defense against potentially hostile userspace.
Currently VFIO can take MMIO regions from the device's BAR and map
them into a PFNMAP VMA with special PTEs. This mapping type ensures
the memory cannot be used with things like pin_user_pages(), hmm, and
so on. In practice only the user process CPU and KVM can safely make
use of these VMA. When VFIO shuts down these VMAs are cleaned by
unmap_mapping_range() to prevent any UAF of the MMIO beyond driver
unbind.
However, VFIO type 1 has an insecure behavior where it uses
follow_pfnmap_*() to fish a MMIO PFN out of a VMA and program it back
into the IOMMU. This has a long history of enabling P2P DMA inside
VMs, but has serious lifetime problems by allowing a UAF of the MMIO
after the VFIO driver has been unbound.
Introduce DMABUF as a new safe way to export a FD based handle for the
MMIO regions. This can be consumed by existing DMABUF importers like
RDMA or DRM without opening an UAF. A following series will add an
importer to iommufd to obsolete the type 1 code and allow safe
UAF-free MMIO P2P in VM cases.
DMABUF has a built in synchronous invalidation mechanism called
move_notify. VFIO keeps track of all drivers importing its MMIO and
can invoke a synchronous invalidation callback to tell the importing
drivers to DMA unmap and forget about the MMIO pfns. This process is
being called revoke. This synchronous invalidation fully prevents any
lifecycle problems. VFIO will do this before unbinding its driver
ensuring there is no UAF of the MMIO beyond the driver lifecycle.
Further, VFIO has additional behavior to block access to the MMIO
during things like Function Level Reset. This is because some poor
platforms may experience a MCE type crash when touching MMIO of a PCI
device that is undergoing a reset. Today this is done by using
unmap_mapping_range() on the VMAs. Extend that into the DMABUF world
and temporarily revoke the MMIO from the DMABUF importers during FLR
as well. This will more robustly prevent an errant P2P from possibly
upsetting the platform.
A DMABUF FD is a preferred handle for MMIO compared to using something
like a pgmap because:
- VFIO is supported, including its P2P feature, on archs that don't
support pgmap
- PCI devices have all sorts of BAR sizes, including ones smaller
than a section so a pgmap cannot always be created
- It is undesirable to waste a lot of memory for struct pages,
especially for a case like a GPU with ~100GB of BAR size
- We want a synchronous revoke semantic to support FLR with light
hardware requirements
Use the P2P subsystem to help generate the DMA mapping. This is a
significant upgrade over the abuse of dma_map_resource() that has
historically been used by DMABUF exporters. Experience with an OOT
version of this patch shows that real systems do need this. This
approach deals with all the P2P scenarios:
- Non-zero PCI bus_offset
- ACS flags routing traffic to the IOMMU
- ACS flags that bypass the IOMMU - though vfio noiommu is required
to hit this.
There will be further work to formalize the revoke semantic in
DMABUF. For now this acts like a move_notify dynamic exporter where
importer fault handling will get a failure when they attempt to map.
This means that only fully restartable fault capable importers can
import the VFIO DMABUFs. A future revoke semantic should open this up
to more HW as the HW only needs to invalidate, not handle restartable
faults.

Make sure that all VFIO PCI devices have peer-to-peer capabilities
enables, so we would be able to export their MMIO memory through DMABUF,
VFIO has always supported P2P mappings with itself. VFIO type 1
insecurely reads PFNs directly out of a VMA's PTEs and programs them
into the IOMMU allowing any two VFIO devices to perform P2P to each
other.
All existing VMMs use this capability to export P2P into a VM where
the VM could setup any kind of DMA it likes. Projects like DPDK/SPDK
are also known to make use of this, though less frequently.
As a first step to more properly integrating VFIO with the P2P
subsystem unconditionally enable P2P support for VFIO PCI devices. The
struct p2pdma_provider will act has a handle to the P2P subsystem to
do things like DMA mapping.
While real PCI devices have to support P2P (they can't even tell if an
IOVA is P2P or not) there may be fake PCI devices that may trigger
some kind of catastrophic system failure. To date VFIO has never
tripped up on such a case, but if one is discovered the plan is to add
a PCI quirk and have pcim_p2pdma_init() fail. This will fully block
the broken device throughout any users of the P2P subsystem in the
kernel.
Thus P2P through DMABUF will follow the historical VFIO model and be
unconditionally enabled by vfio-pci.

Instead of hooking the general ioctl op, have the core code directly
decode VFIO_DEVICE_GET_REGION_INFO and call an op just for it.
This is intended to allow mechanical changes to the drivers to pull their
VFIO_DEVICE_GET_REGION_INFO int oa function. Later patches will improve
the function signature to consolidate more code.

This was missed during the initial implementation. The VFIO PCI encodes
the vf_token inside the device name when opening the device from the group
FD, something like:

This is used to control access to a VF unless there is co-ordination with
the owner of the PF.
Since we no longer have a device name in the cdev path, pass the token

indicated by VFIO_DEVICE_BIND_FLAG_TOKEN.

In the below noted Fixes commit we introduced a reflck mutex to allow
better scaling between devices for open and close. The reflck was
based on the hot reset granularity, device level for root bus devices
which cannot support hot reset or bus/slot reset otherwise. Overlooked
in this were SR-IOV VFs, where there's also no bus reset option, but
the default for a non-root-bus, non-slot-based device is bus level
reflck granularity.
The reflck mutex has since become the dev_set mutex (via commit
2cd8b14aaa ("vfio/pci: Move to the device set infrastructure")) and
is our defacto serialization for various operations and ioctls. It
still seems to be the case though that sets of vfio-pci devices really
only need serialization relative to hot resets affecting the entire
set, which is not relevant to SR-IOV VFs. As described in the Closes
link below, this serialization contributes to startup latency when
multiple VFs sharing the same "bus" are opened concurrently.
Mark the device itself as the basis of the dev_set for SR-IOV VFs.

PFN_DEV was used by callers of dax_direct_access() to figure out if the
returned PFN is associated with a page using pfn_t_has_page() or not.
However all DAX PFNs now require an assoicated ZONE_DEVICE page so can
assume a page exists.
Other users of PFN_DEV were setting it before calling vmf_insert_mixed().
This is unnecessary as it is no longer checked, instead relying on
pfn_valid() to determine if there is an associated page or not.

The vfio-pci huge_fault handler doesn't make any attempt to insert a
mapping containing the faulting address, it only inserts mappings if the
faulting address and resulting pfn are aligned. This works in a lot of
cases, particularly in conjunction with QEMU where DMA mappings linearly
fault the mmap. However, there are configurations where we don't get
that linear faulting and pages are faulted on-demand.
The scenario reported in the bug below is such a case, where the physical
address width of the CPU is greater than that of the IOMMU, resulting in a
VM where guest firmware has mapped device MMIO beyond the address width of
the IOMMU. In this configuration, the MMIO is faulted on demand and
tracing indicates that occasionally the faults generate a VM_FAULT_OOM.
Given the use case, this results in a "error: kvm run failed Bad address",
killing the VM.
The host is not under memory pressure in this test, therefore it's
suspected that VM_FAULT_OOM is actually the result of a NULL return from
__pte_offset_map_lock() in the get_locked_pte() path from insert_pfn().
This suggests a potential race inserting a pte concurrent to a pmd, and
maybe indicates some deficiency in the mm layer properly handling such a
case.
Nevertheless, Peter noted the inconsistency of vfio-pci's huge_fault
handler where our mapping granularity depends on the alignment of the
faulting address relative to the order rather than aligning the faulting
address to the order to more consistently insert huge mappings. This
change not only uses the page tables more consistently and efficiently, but
as any fault to an aligned page results in the same mapping, the race
condition suspected in the VM_FAULT_OOM is avoided.

The ability to map PCI resources to user-space is controlled by global
defines. For vfio there is VFIO_PCI_MMAP which is only disabled on s390 and
controls mapping of PCI resources using vfio-pci with a fallback option via
the pread()/pwrite() interface.
For the PCI core there is ARCH_GENERIC_PCI_MMAP_RESOURCE which enables a
generic implementation for mapping PCI resources plus the newer sysfs
interface. Then there is HAVE_PCI_MMAP which can be used with custom
definitions of pci_mmap_resource_range() and the historical /proc/bus/pci
interface. Both mechanisms are all or nothing.
For s390 mapping PCI resources is possible and useful for testing and
certain applications such as QEMU's vfio-pci based user-space NVMe driver.
For certain devices, however access to PCI resources via mappings to
user-space is not possible and these must be excluded from the general PCI
resource mapping mechanisms.
Introduce pdev->non_mappable_bars to indicate that a PCI device's BARs can
not be accessed via mappings to user-space. In the future this enables
per-device restrictions of PCI resource mapping.
For now, set this flag for all PCI devices on s390 in line with the
existing, general disable of PCI resource mapping. As s390 is the only user
of the VFI_PCI_MMAP Kconfig options this can already be replaced with a
check of this new flag. Also add similar checks in the other code protected
by HAVE_PCI_MMAP respectively ARCH_GENERIC_PCI_MMAP in preparation for
enabling these for supported devices.

Some systems report INTx as not routed by setting pdev->irq to
IRQ_NOTCONNECTED, resulting in a -ENOTCONN error when trying to
setup eventfd signaling. Include this in the set of conditions
for which the PIN register is virtualized to zero.
Additionally consolidate vfio_pci_get_irq_count() to use this
virtualized value in reporting INTx support via ioctl and sanity
checking ioctl paths since pdev->irq is re-used when the device
is in MSI mode.
The combination of these results in both the config space of the
device and the ioctl interface behaving as if the device does not
support INTx.

With the addition of pfnmap support in vmf_insert_pfn_{pmd,pud}() we can
take advantage of PMD and PUD faults to PCI BAR mmaps and create more
efficient mappings. PCI BARs are always a power of two and will typically
get at least PMD alignment without userspace even trying. Userspace
alignment for PUD mappings is also not too difficult.
Consolidate faults through a single handler with a new wrapper for
standard single page faults. The pre-faulting behavior of commit
d71a989cf5 ("vfio/pci: Insert full vma on mmap'd MMIO fault") is removed
in this refactoring since huge_fault will cover the bulk of the faults and
results in more efficient page table usage. We also want to avoid that
pre-faulted single page mappings preempt huge page mappings.

The "array_count" value comes from the copy_from_user() in
vfio_pci_ioctl_pci_hot_reset(). If the user passes a value larger than
INT_MAX then we'll pass a negative value to kcalloc() which triggers an
allocation failure and a stack trace.
It's better to make the type unsigned so that if (array_count > count)
returns -EINVAL instead.

In order to improve performance of typical scenarios we can try to insert
the entire vma on fault. This accelerates typical cases, such as when
the MMIO region is DMA mapped by QEMU. The vfio_iommu_type1 driver will
fault in the entire DMA mapped range through fixup_user_fault().
In synthetic testing, this improves the time required to walk a PCI BAR
mapping from userspace by roughly 1/3rd.
This is likely an interim solution until vmf_insert_pfn_{pmd,pud}() gain
support for pfnmaps.

With the vfio device fd tied to the address space of the pseudo fs
inode, we can use the mm to track all vmas that might be mmap'ing
device BARs, which removes our vma_list and all the complicated lock
ordering necessary to manually zap each related vma.
Note that we can no longer store the pfn in vm_pgoff if we want to use
unmap_mapping_range() to zap a selective portion of the device fd
corresponding to BAR mappings.
This also converts our mmap fault handler to use vmf_insert_pfn()
because we no longer have a vma_list to avoid the concurrency problem
with io_remap_pfn_range(). The goal is to eventually use the vm_ops
huge_fault handler to avoid the additional faulting overhead, but
vmf_insert_pfn_{pmd,pud}() need to learn about pfnmaps first.
Also, Jason notes that a race exists between unmap_mapping_range() and
the fops mmap callback if we were to call io_remap_pfn_range() to
populate the vma on mmap. Specifically, mmap_region() does call_mmap()
before it does vma_link_file() which gives a window where the vma is
populated but invisible to unmap_mapping_range().

Yi notes relative to commit f6944d4a0b ("vfio/pci: Collect hot-reset
devices to local buffer") that we previously tested the resulting
device count with a WARN_ON, which was removed when we switched to
the in-loop user copy in commit b56b7aabcf ("vfio/pci: Copy hot-reset
device info to userspace in the devices loop"). Finding no devices in
the bus/slot would be an unexpected condition, so let's restore the
warning and trigger a -ERANGE error here as success with no devices
would be an unexpected result to userspace as well.

The VM_ALLOW_ANY_UNCACHED flag is implemented for ARM64,
allowing KVM stage 2 device mapping attributes to use Normal-NC
rather than DEVICE_nGnRE, which allows guest mappings supporting
write-combining attributes (WC). ARM does not architecturally
guarantee this is safe, and indeed some MMIO regions like the GICv2
VCPU interface can trigger uncontained faults if Normal-NC is used.
To safely use VFIO in KVM the platform must guarantee full safety
in the guest where no action taken against a MMIO mapping can
trigger an uncontained failure. The expectation is that most VFIO PCI
platforms support this for both mapping types, at least in common
flows, based on some expectations of how PCI IP is integrated. So
make vfio-pci set the VM_ALLOW_ANY_UNCACHED flag.

The VFIO_DEVICE_GET_INFO, VFIO_DEVICE_GET_REGION_INFO, and
VFIO_IOMMU_GET_INFO ioctls fill in an info struct followed by capability
structs:

Both the info and capability struct sizes are not always multiples of
sizeof(u64), leaving u64 fields in later capability structs misaligned.
Userspace applications currently need to handle misalignment manually in
order to support CPU architectures and programming languages with strict
alignment requirements.
Make life easier for userspace by ensuring alignment in the kernel. This
is done by padding info struct definitions and by copying out zeroes
after capability structs that are not aligned.
The new layout is as follows:

In this example caps[0] has a size that is not multiples of sizeof(u64),
so zero padding is added to align the subsequent structure.
Adding zero padding between structs does not break the uapi. The memory
layout is specified by the info.cap_offset and caps[i].next fields
filled in by the kernel. Applications use these field values to locate
structs and are therefore unaffected by the addition of zero padding.
Note that code that copies out info structs with padding is updated to
always zero the struct and copy out as many bytes as userspace
requested. This makes the code shorter and avoids potential information
leaks by ensuring padding is initialized.

This copies the vfio_pci_dependent_device to userspace during looping each
affected device for reporting vfio_pci_hot_reset_info. This avoids counting
the affected devices and allocating a potential large buffer to store the
vfio_pci_dependent_device of all the affected devices before copying them

This allows VFIO_DEVICE_GET_PCI_HOT_RESET_INFO ioctl use the iommufd_ctx
of the cdev device to check the ownership of the other affected devices.
When VFIO_DEVICE_GET_PCI_HOT_RESET_INFO is called on an IOMMUFD managed
device, the new flag VFIO_PCI_HOT_RESET_FLAG_DEV_ID is reported to indicate
the values returned are IOMMUFD devids rather than group IDs as used when
accessing vfio devices through the conventional vfio group interface.
Additionally the flag VFIO_PCI_HOT_RESET_FLAG_DEV_ID_OWNED will be reported
in this mode if all of the devices affected by the hot-reset are owned by
either virtue of being directly bound to the same iommufd context as the
calling device, or implicitly owned via a shared IOMMU group.

vfio_pci_is_device_in_set() now returns -EBUSY in commit a882c16a2b

Not all MSI-X devices support dynamic MSI-X allocation. Whether
a device supports dynamic MSI-X should be queried using
pci_msix_can_alloc_dyn().
Instead of scattering code with pci_msix_can_alloc_dyn(),
probe this ability once and store it as a property of the
virtual device.

Interrupt context is statically allocated at the time interrupts
are allocated. Following allocation, the context is managed by
directly accessing the elements of the array using the vector
as index. The storage is released when interrupts are disabled.
It is possible to dynamically allocate a single MSI-X interrupt
after MSI-X is enabled. A dynamic storage for interrupt context
is needed to support this. Replace the interrupt context array with an
xarray (similar to what the core uses as store for MSI descriptors)
that can support the dynamic expansion while maintaining the
custom that uses the vector as index.
With a dynamic storage it is no longer required to pre-allocate
interrupt contexts at the time the interrupts are allocated.
MSI and MSI-X interrupt contexts are only used when interrupts are
enabled. Their allocation can thus be delayed until interrupt enabling.
Only enabled interrupts will have associated interrupt contexts.
Whether an interrupt has been allocated (a Linux irq number exists
for it) becomes the criteria for whether an interrupt can be enabled.

Use GFP_KERNEL_ACCOUNT for userspace persistent allocations.
The GFP_KERNEL_ACCOUNT option lets the memory allocator know that this
is untrusted allocation triggered from userspace and should be a subject
of kmem accounting, and as such it is controlled by the cgroup
mechanism.
The way to find the relevant allocations was for example to look at the
close_device function and trace back all the kfrees to their
allocations.
